From the published article (and the article seems to give more technical info than they usually do) it sounds like this guy broke more than a couple of the BASIC rules in diving. It seems to have been a pretty easily avoidable death had "the rules" been followed. I'm curious as to why "the investigators" were unable to discern what the "caked powder" in the loop actually was.......

Good point about the flooding issue. I hadn't considered that. I still don't understand who was "investigating" and why they couldn't figure out what the powder really was. Any other info on this incident on Rebreather World or any other sites you frequent?
