I do have one . It works well takes a little getting used to . The position on your head is different. LOVE it for colder water

How do you wear it with a hood? seems like you would have to put it on before you donned the hood, but the hood would be difficult to don an seal.

They (IST) make a special hood (sold seperatly). The hood is 2 piece the inner hood goes on your head , and has cut outs for the ear cups. The outer hood goes over everything.So its hood,fold over outer shell , mask with cups thru cutouts , outer hood. Down side is the hoods are short unless they have been changed since I got mine

Does it help with ear problems as it claims? I at times can have difficulty clearing/equalizing my ear, leading to an uncomfortable dive.

It does seem to help. I got mine for that reason . The first time I dove it in cold water I was sold
